Caremed Inc. is hiring a Patient Care Coordinator Home Care to coordinate patient care, caregiver schedules, home care services, EVV, and daily client scheduling operations.
This fast-paced role involves patient scheduling, caregiver assignments, inbound/outbound calls, EVV, visit verification, cancellations, call-outs, missed visits, reassignments, and staffing coverage.
Caremed Inc. provides and coordinates in-home care services throughout Central Virginia, including Amelia, Charlotte, Chesterfield, Appomattox, Buckingham, and Henrico Counties, as well as Richmond City, Hopewell, Colonial Heights, and Petersburg. Patient Care Coordinators help ensure reliable scheduling and coverage across our service area.
