The Opportunity

If you became an FNP because you wanted to know your patients — their families, their histories, their goals — outpatient primary care in Wayne County gives you that. This is a community practice where you see the same families across life stages, and where your work is visible to the people you serve.

The compensation is stated up front: $115,000 to $135,000, with relocation assistance covered for providers coming from outside the area. This is an outpatient primary care role with no remote component.

Your Day-to-Day

• Provide outpatient primary care to a family patient population across life stages
• Build long-term relationships with patients and families in a close-knit community
• Work collaboratively within a care team connected to a broader regional system
• Document and coordinate care through shared clinical systems used across the affiliated network
• Support preventive care and ongoing management for a population that values accessible care close to home

What We're Looking For

• Master's degree — Nurse Practitioner
• Board-Certified Family Nurse Practitioner
• Background in primary care, family medicine, internal medicine, community health, or outpatient practice
• Interest in long-term patient relationships and collaborative team-based care
• Willingness to live in or relocate to Wayne County, Geneva, the northern Finger Lakes, or the Rochester/Syracuse corridor
• No visa sponsorship available for this position
