Hospice Admissions Nurse — Care Coord & On-Call
Valor Hospice
Valor Hospice is seeking a Registered Nurse to perform admissions and provide nursing care in patients' homes in the Houston area. You will coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agencies while managing documentation and on-call duties. Qualified candidates have an RN license, 2 years of clinical RN experience, and at least 1 year in hospice.
