Job Title Licensed Practical Nurse (LPN) Locations and Schedules Clarks Summit, PA; Scott Township, PA; Nicholson, PA. Immediate need: Clarks Summit – all shifts available 24 hours a day. Scott Township – Monday to Friday 7:15 a.m. to 4:45 p.m. Nicholson – Monday to Friday 8:20 a.m. to 2:45 p.m. Factoryville – 7 days a week, 16 hours a day. Rates: $28.00–$32.00 per hour. Job Responsibilities One-to-one patient care in the home. Implement and document the plan of care to ensure quality and continuity of care. Administer prescribed medication, treatments and therapies. Patient assessments and coordination of care. Report changes in client condition to Clinical Manager and physician. Health promotion, teaching, and training of family members.
