PT/PRN LPN/RN

Job Summary:

Riverstone Post Acute, a leading healthcare provider, is seeking a dedicated and compassionate PT or PRN LPN/RN to join our team in Salina, KS. As a PT or PRN LPN/RN, you will play a vital role in providing high-quality patient care to our residents. Responsibilities:

• Provide skilled nursing care to residents, including medication administration, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Maintain accurate and up-to-date records of resident care and services

• Collaborate with other healthcare professionals to ensure seamless care transitions

• Participate in Quality Improvement initiatives to enhance resident outcomes and satisfaction

• Maintain confidentiality and adhere to HIPAA guidelines

Requirements:

• Current LPN/RN license in the state of Kansas

• 0 minimum years of experience

• Ability to work variable shifts, including evenings, weekends, and holidays

• Strong communication and interpersonal skills

• Compassionate and empathetic approach to patient care
