Registered Nurse - Cardiovascular

Role Overview: Provides comprehensive care for patients with chronic or acute heart failure in an outpatient clinic setting. Key Responsibilities Assess patients through physical exams, symptom review, and vital sign monitoring. Educate patients and families on heart failure management, medication adherence, and lifestyle modifications. Coordinate care plans with healthcare providers, including medication titration per protocols. Manage incoming calls to evaluate symptoms, modify care, or refer to physicians. Act as a liaison between physicians and patients, facilitating follow-ups and diagnostic tests. Qualifications & Skills Licensed Registered Nurse credentialed from the applicable state nursing board or eligible for licensure. BLS certification obtained within 30 days of hire, accepted from AHA or ARC. Diploma from an accredited nursing school or equivalent professional licensure at hiring. Key Attributes: Strong patient assessment, education, coordination skills, and ability to act as a healthcare liaison.
