Our Company Rehab Without Walls Neuro Rehabilitation Overview Rehab Without Walls is hiring a Licensed Clinical Social Worker (LCSW) to join our Home & Community Rehabilitation team in San Antonio,TX! Who we are looking for: * We're seeking a LCSW to support individuals with neurological injuries such as stroke and traumatic brain injury (TBI). * In this vital role, you'll provide therapeutic interventions to promote emotional and social adjustment, while collaborating with the rehab team to maximize patient outcomes and help overcome barriers to progress. * Prefer 2+ years of experience with knowledge of neurological injuries and/or medical social work background What will you get: * Flexible Schedule Created by You * Paid per hour (not just per visit) * No weekends, evenings, or holidays * Drive Time Reimbursed at Clinical Rate & Mileage Reimbursement * Collaborative Team Environment * Educational Programs * Growth/Advancement Opportunities Responsibilities What you will do: (responsibilities listed are included but not limited to) * Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families * Sets measurable objectives that are formulated in conjunction with the rehab team * Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems * Instructs/counsels' patients and families in treating and coping with social and emotional response connected with illnesses * Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs * Identifies and assists the patient and/or family to utilize appropriate community resources to achieve measurable objectives Qualifications What you'll need: * Master's Degree in Clinical Social Work (LCSW) * Current Licensed Clinical Social Worker Licensure in the state of practice * Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis * Current BLS (Basic Life Support) First Aid and CPR certification required * Ability to lift 50 pounds * Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures * Duties require fine motor skills, visual acuity, and walking/ standing for extended periods * Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times About our Line of Business Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
