Physical Therapist (PT) - Home Health

TLC

Home Health Nurse Skilled Nursing

Self-scheduled visits based on office needs, with typical visits of 68 per day and weekends required for 12 shifts per contract.

Standard caseload of 68 visits per day.

Requirements

Minimum of 1 year of experience in home health
Valid state license (FL or compact license required at time of submission)
BLS certification
OASIS training preferred
Reliable transportation for home visits
Experience with diverse patient conditions including med/surg, cardiac, ortho, COPD, and post-surgical cases

Responsibilities

Perform patient assessments and develop treatment plans
Deliver cardiopulmonary care and manage orthopedics/post-op recovery
Provide neuro care, including rehabilitation for stroke and Parkinson's patients
Educate patients and families on safety, injury prevention, and activities of daily living (ADLs)
Utilize assistive devices for patient mobility and training
Ensure accurate documentation and billing practices related to home health care

Compliance / Notes

COVID-19 vaccination required, with exemptions considered; flu shot required or declination; compliance documentation due prior to start date.
