Senior Community Health NurseYour specific responsibilities include:Offers float support in all clinical areas within community and clinical health spaceProvides direct nursing care in the Florida Breast and Cervical Cancer Early Detection Program (FBCC) and Maternity/OB clinics as assigned based on daily staffing needsProvides direct nursing care in the STD, HIV/Ryan White, and Same Day Testing clinics as assigned based on daily staffing needsConducts sexual and risk history, symptom assessment, and specimen collection per CDC STI Treatment Guidelines and Department protocolsAdministers treatment and expedited partner therapy where permitted, per standing orders and provider directionPerforms rapid HIV, syphilis, and hepatitis C point-of-care testing with pre-test and post-test counseling per Florida Department of Health counseling and testing standardsDelivers preliminary positive results and coordinates same-day confirmatory testing and rapid linkage to care, including rapid antiretroviral initiation when availableFacilitates same-day PrEP and nPEP assessment, baseline labs, and provider handoffSupports HIV primary care visits, including vital signs, medication reconciliation, adherence assessment, and laboratory collection (viral load, CD4, and resistance testing as ordered)Reinforces antiretroviral adherence education and assists with Ryan White eligibility and recertification documentation as assignedPerforms CLIA-waived testing quality control and maintains testing logs per laboratory policyFollows up on abnormal results to ensure clients receive diagnostic services and treatment as ordered by the attending providerRefers eligible FBCC clients to the Mary Brogan Breast and Cervical Cancer Medicaid pathway for treatmentCoordinates syphilis and congenital syphilis follow-up with Disease Intervention Specialists (DIS), including pregnancy status screeningMonitors missed appointments and laboratory results and initiates re-engagement outreach for clients out of care or not virally suppressedTracks high-risk prenatal referrals and coordinates with delivering hospitals, case managers, and community partnersRefers clients to community resources as neededCompletes standardized handoff of all pending follow-up before rotating to another clinic assignmentDocuments all services accurately and timely in the Health Management System (HMS), Florida SHOTS, and program data systemsCompletes reportable disease documentation and reporting per Chapter 64D-3, Florida Administrative CodeEnsures program forms and data required for FBCC, Ryan White, STD, Title X, and maternity reporting are complete and submitted per program requirementsMaintains confidentiality of client health information per HIPAA, Section 381.004, Florida Statutes (HIV), Section 384.29, Florida Statutes (STD), and Department information security policyReports suspected abuse, neglect, or exploitation per Chapters 39 and 415, Florida StatutesParticipates in chart audits, program site reviews, and corrective action plansRequired knowledge, skills, and abilities include:Ability to be flexible regarding work schedules and locationsKnowledge of nursing principles, practices, and techniquesKnowledge of human anatomy and physiologyAbility to accurately interpret Family Planning, OB, 540B, Ryan White, and FBCC guidelines and regulations for service delivery and disseminate the information to coworkers and providersAbility to accurately complete complex paperwork associated with Community and Clinical Health SpaceAbility to prepare clear and concise reportsAbility to effectively educate providers about the program guidelines, eligibility requirements, billing, etc.Ability to effectively educate the public about Clinical Breast Exams (CBE), monthly self-checks, mammograms & Papanicolaou (PAP), and to inform eligible women on program requirementsAbility to manage and track budgetAbility to communicate effectively and courteously in a culturally sensitive mannerAbility to maintain effective working relationships with othersAbility to solve problems creativelyAbility to prioritize and organize tasks for maximum efficiencyBasic computer skillsQualifications:Minimum – Licensure as a Registered Nurse in accordance with Florida Statute 464Valid driver's licenseKnowledge of compliance regarding state-sponsored programsPreferred – Bachelor's degree of higherHistory and background in one or more of the following clinical programs: OB, Family Planning, Female Health, FBCCP, STD/HIV careWhere you will work:William L.
