Registered Nurse Case Manager

Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Case Manager, Hospice to provide end‑of‑life care to patients in their homes in the Cheswick, PA area. The position is eligible for a $10,000 first‑year success bonus. Employment Type: Staff Shift: 8 hours Duration: Ongoing 40 hours per week Responsibilities Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning. Educate and support patients and families through the end‑of‑life journey, ensuring they feel informed, prepared, and deeply cared for. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently, but never alone. Travel to patient homes within the assigned territory. Qualifications Active Registered Nurse (RN) license in the state of work. Two years of RN experience and the clinical expertise to deliver quality patient care; hospice experience preferred.
