Social Worker MSW - Care Management - HPW - FT - Day

Position Overview Pay Range: $34.99 – $45.71 per hour (full‑time equivalent 1.0 FTE) Scheduled Weekly Hours: 40 hours FLSA Exemption Status: Non‑Exempt Summary: Assesses, diagnoses, intervenes with, and plans for social work services related to patient and family bio‑psycho‑social needs and behaviors. Integrates assessment using information related to cultural, clinical, ethnic, age and other factors into a plan of care to implement transition plans and treatments. Acts as an active team member in the discharge planning process, identifying complicated social and financial factors that are barriers to appropriate discharge and ensuring patients are referred to appropriate social and financial resources post‑discharge. Provides referrals regarding abuse/neglect/domestic violence, alcohol and substance abuse, and performs clinical interventions related to grief counseling, bereavement, adjustment, and crisis intervention. Minimum Requirements Education: Master’s degree in Social Work (MSW) Experience: One year of clinical social work experience preferred Credentials: Licensed Social Worker Knowledge & Skills: Excellent communication, conflict management, organizational and planning skills Special Training: Clinical training in a medical/clinical environment; knowledge of spreadsheet, word processing, case management and patient information systems Usual Work Day: 8 hours Essential Functions (Responsibilities) Document clinical records, comprehensive assessments, interventions, support, and community resources in a recognized format; include assessments, plans, and referrals in the discharge planning process. Use culturally competent skills to assess individual patient needs and refer to appropriate professional staff when necessary; identify barriers and plan interventions to achieve outcomes. Collaborate, educate, and assist the health care team to identify social and financial barriers that delay appropriate transitions through multidisciplinary rounds, length of stay meetings, and documentation. Document all referrals and clinical interventions related to abuse, neglect, domestic violence, alcohol, and substance abuse; provide educational sessions to staff regarding reporting of these issues. Counsel staff and patients/families on conflict management; monitor reactions and needs for ongoing intervention; complete mental health/psych assessments when appropriate. Advocate for appropriate services for all patients with payor and providers of post‑acute care, community services, and other services to optimize outcomes. Participate in staff development and performance improvement programs to increase knowledge, staff and colleague awareness of bio‑psycho‑social issues, improve departmental systems, and meet regulatory requirements. Attend and participate in regular departmental performance programs, staff meetings, and hospital‑wide committees to analyze and respond to issues significant to social work services and identify opportunities for improvement. Assist in data collection and information gathering on patient care issues, program and systems analysis, to comply with JCAHO’s Plan‑Do‑Check‑Act performance improvement standards. Develop inpatient and community programs that benefit patients, families, and staff; implement support groups for specified disease processes or bio‑psycho‑social issues; perform follow‑up services as needed to measure clinical outcomes. Perform other duties as assigned. Physical Demands and Work Environment Frequent demands include standing, walking, carrying objects, and keyboard use. Occasional demands include sitting, climbing stairs or ladders, pushing/pulling, twisting, bending, and reaching forward or overhead. Continuous physical demands: lifting floor to waist 15 lbs, lifting waist level and above 10 lbs. Sensory requirements: accurate near and far vision, color discrimination, minimal depth perception, and accurate hearing. Occupational exposure risks: bloodborne pathogens, airborne communicable disease.
