Contract | Full-Time | Days | Monday–Friday, 8:00 AM–5:00 PM | 13-Week Assignment | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting within San Bernardino County
• Conduct thorough patient evaluations and develop individualized treatment plans
• Complete OASIS assessments accurately and in accordance with regulatory requirements
• Document patient progress and clinical notes using the HCHB electronic medical records system
• Meet productivity expectations of 30 points per 40-hour work week
• Collaborate with the interdisciplinary care team to ensure coordinated, patient-centered care

Required Qualifications

• Active and unrestricted Physical Therapist (PT) license in the state of California
• Minimum of 1 year of home health experience
• Minimum of 1 year of OASIS experience
• Proficiency with HCHB (Homecare Homebase) EMR system
• Valid driver's license and reliable transportation for patient home visits within San Bernardino County

Preferred Experience / Skills

• Experience managing a high-volume caseload in a home health environment
• Strong documentation and time-management skills in a field-based setting
