Pride Health is hiring an Occupational Therapist (OT) - Home Health to support our client's healthcare facility in Easton, MD.

Job Details: Schedule: Monday-Friday, 8:00 AM - 4:30 PM (40 Hours Per Week)

Location: Easton, MD 21601

Float / Coverage Area: Talbot, Dorchester, Caroline, Queen Anne's, and Kent Counties, Maryland

Job Type: Local contract

Contract Length: 14 Weeks Pay Range: $50.00 to $53.00/hour*

Mileage Reimbursement Rate (if applicable): $0.51/ mile *Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Perform comprehensive Occupational Therapy evaluations and OASIS-driven assessments in the home setting.
• Complete initial evaluations, routine patient visits, reassessments, recertifications, and discharge assessments.
• Develop, implement, and update individualized Plans of Care (POC) based on physician orders and patient-specific needs.
• Assess and address deficits related to activities of daily living (ADLs), mobility, safety, cognition, strength, coordination, and functional independence.
• Recommend adaptive equipment, environmental modifications, and therapeutic interventions to maximize patient safety and independence.
• Educate patients, caregivers, and family members on home safety, adaptive techniques, therapeutic exercises, and energy conservation strategies.
• Collaborate closely with nurses, physical therapists, speech therapists, physicians, case managers, and other interdisciplinary team members.
• Maintain timely and accurate clinical documentation and OASIS requirements in accordance with agency, state, and federal regulations.
• Communicate effectively with patients, caregivers, providers, and office staff to coordinate high-quality patient care.
• Meet established productivity standards while delivering exceptional patient-centered care.

Requirements:

• Active Maryland Occupational Therapist License in good standing.
• Previous Home Health experience required.
• Strong OASIS assessment experience required.
