This will be a 12-month, 20 hour per week position.
The Wellness Case Manager will be charged with assisting Counseling and Health Services, by addressing the needs of students. This can include: health, mental well-being, economic, food insecurities, and/or academic concerns through a variety of interventions, referrals and follow-up services. Responsibilities will include case management, programming, collaboration and consultation with faculty, staff, athletics, off-campus stakeholders, parents/families, other campus stakeholders and colleagues within the Counseling and Health Services Departments. This position will work to foster student development and well-being by offering a supportive, personalized response when difficulties arise. Reports to Director of Counseling Services.
Job Responsibilities:
Build relationships with campus and off-campus resources
Refer students to campus/off-campus resources
Accurate and timely record keeping and data collection
Maintain strict confidentiality
Positive outlook and willingness to work through obstacles
Flexible in schedule and thinking
Direct skills teaching
Facilitate small groups
Recognize signs and symptoms of mental health crisis
Work closely with Counseling Services and Health Services
Developing and implementing appropriate, educational programming
Responsible for client care including: planning, implementing, monitoring, and making referral of actions required to meet the clients’ health and human services needs
Working with offsite support, coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
Maintain caseload and productivity by maintaining regular contact with clients to monitor and assess progress
Deliver on-going services to clients which may include: crisis intervention, advocacy, budgeting, housing assistance, and referral and linkage to community resources as needed
Participate in treatment team and assist in making recommendations for client status changes
May lead educational groups and/or facilitate group curriculum
Adhere to professional standards as outlined by protocols, rules and regulations
Other duties as assigned
Requirements:
Bachelor’s Degree in counseling, social work, student affairs or related field
Demonstrated interest in and experience working with college students including diverse and/or underrepresented college student populations
Experience in crisis intervention, experience in mental health work required
Ability to work as part of a team
Basic phone and fax skills
Excellent organizational and time management skills
Effective verbal and written communication skills
Advanced level problem solving skills and ability to multi-task while prioritizing
Basic computer skills with ability to navigate Microsoft applications including but not limited to Word, Outlook, Excel, Teams
