Full time Certified Home Health Aide (CHHA) 

Territory: Santa Rosa & Surrounding Cities

Under the direct supervision of an RN Case Manager, you will provide patient care according to the Plan of Care.

Estimated Wage range Certified Home Health Aide (CHHA): 55,000.00 - 90,000.00 Starting pay is commensurate with relevant experience above the minimum requirements.  21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match)
