Physical Therapist

Schedule: 8 hours per day

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 13

Requirements:

• Current State License as a Physical Therapist - Required
• Proof of current CPR certification - Required
• Current driver's license. Must have reliable transportation and insurance - Required
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document and review patient care plans, progress notes, and other clinical documentation on an ongoing basis.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on evaluations.
• Instruct patients and families on safe transfer techniques and the use of therapy equipment.
• Provide patient/family education regarding therapeutic procedures tailored to the patient's needs.
• Ensure compliance with infection control standards and coordinate patient care activities.
