(LFCD) is a community development non‑profit agency established in the City of Richmond in Contra Costa County in 1980. Today Lao Family has expanded its operations and service footprint to two additional counties, including Alameda and Sacramento. LFCD’s headquarters office is in Oakland, CA. It delivers programs and services from 7 locations in 35 languages. The agency provides both community development real estate facilities and a diverse array of workforce, education, and human services that directly support predominantly low‑income US‑born high‑barrier families and individuals, refugees, immigrants, transitional age youth, seniors, and other special populations such as individuals with disabilities. Job Summary: Under the direction of the program supervisor, this position will serve low‑income Workforce Innovation Opportunity Act (WIOA) participants to gain job placements, as well as manage participants under the California Advancing and Innovating Medical (Cal‑AIM) Initiative, which provides comprehensive care management to address both clinical and non‑clinical needs. This position is responsible for interfacing with local employers and training providers, external organizations, and the internal LFCD department, especially the America’s Job Center of California (AJCC) and Business Centers. This position is required to rotate at Berkeley Adult School and LFCD AJCC locations and reports to the Program Supervisor. Roles and Responsibilities: Conduct comprehensive participant assessments to develop individualized care management and employment plans that address physical health, behavioral health, substance use disorders, oral health, social determinants of health, career goals, and employment objectives. Perform community outreach and engagement activities, including attending resource fairs, distributing promotional materials, recruiting participants, and coordinating referrals to workforce programs, healthcare providers, and social service agencies. Facilitate workshops, program orientations, one‑on‑one and group counseling sessions, and educational activities focused on job readiness, career development, healthy living, self‑sufficiency, and goal achievement. Assist participants in developing resumes, cover letters, and Individual Employment Plans (IEPs). Provide labor market information, career counseling, interview preparation, job referrals, job placement assistance, and retention services. Provide intensive case management services, including intake, eligibility determination, enrollment, assessment, documentation collection, career planning, referral coordination, and follow‑up services. Support participants throughout their program participation and provide up to 12 months of employment retention services. Coordinate with training providers to facilitate enrollment in approved training programs and ensure Individual Training Account (ITA) requests and related documentation are submitted accurately and promptly. Maintain accurate participant records, including daily activity logs, case notes, service documentation, assessments, eligibility records, and data entry into CalJOBS, Care Management Systems (CMS), and other required databases. Deliver Community Supports services designed to address health‑related social needs and improve participants’ overall health, stability, and employment outcomes. Schedule and coordinate appointments; provide reminders; arrange transportation; accompany participants to critical medical appointments, community service appointments, and job interviews when appropriate; and assist in resolving barriers to participation. Develop and maintain strong partnerships with employers, healthcare providers, community‑based organizations, educational institutions, and workforce development agencies to expand opportunities and resources for participants. Identify employment opportunities that offer competitive wages and benefits, conduct employer outreach and orientations, maintain employer relationships, and coordinate On‑the‑Job Training (OJT) opportunities. Assist participants in accessing public benefits and supportive services, including Supplemental Security Income (SSI), CalFresh, cash assistance programs, workforce training resources, housing resources, and other community‑based services. Provide transitional care coordination and employment retention services through participant follow‑up, employer engagement, job tracking, and ongoing support to promote long‑term employment success. Maintain current knowledge of Workforce Innovation and Opportunity Act (WIOA), CalAIM, and related workforce and healthcare initiatives. Utilize community partnerships and available resources to improve participant outcomes. Ensure compliance with all applicable federal, state, county, and agency regulations, including participant confidentiality requirements, HIPAA standards, workforce development regulations, and funding source requirements. Collaborate with colleges, career centers, training providers, healthcare organizations, and county agencies to facilitate participant enrollment, training approvals, service coordination, and successful outcomes. Communicate regularly with county departments, workforce development staff, and community partners to ensure program compliance, timely reporting, eligibility verification, and coordinated service delivery. Prepare and submit reports, case notes, performance outcomes, and other program documentation in accordance with agency, county, state, and funding source requirements. Provide administrative and programmatic support to Directors, Managers, Program Supervisors, and Site Supervisors as needed. Perform other duties and special projects as assigned. Requirements and Qualifications: Bachelor’s degree in Social Work, Psychology, Human Services, Business Administration, Communications, Public Health, or a related field required. Instead of a bachelor’s degree, a minimum of one (1) year of experience providing case management, workforce development, social services, healthcare navigation, or related services to vulnerable or underserved populations may be considered. Bilingual proficiency in Spanish and English is highly desirable. Self‑motivated with the ability to work independently with minimal supervision while effectively managing multiple priorities and deadlines. Excellent verbal and written communication, community relations, networking, presentation, and public speaking skills. Demonstrated ability to work effectively and respectfully with individuals and families from diverse cultural, racial, ethnic, socioeconomic, religious, gender, age, disability, and sexual orientation backgrounds. Ability to understand, interpret, explain, and apply complex local, state, and federal laws, regulations, policies, procedures, and program requirements. Strong interpersonal skills with the ability to establish and maintain effective working relationships with participants, employers, healthcare providers, community partners, educational institutions, and government agencies. Ability to travel throughout the service area to meet with participants, employers, healthcare providers, and community partners. Must be available to work occasional evenings and weekends as required by program operations.
