Job Title Registered Nurse (RN) Home Health About the Role We are looking for a home health registered nurse case manager to lead and collaborate in the delivery of patient care plans for patients in Northern Illinois. The RN will support patients, manage care plans, and coordinate services across a team‑based environment. Job Details Duration: Ongoing Hours: 40 per week, 8‑hour shifts Employment type: Staff Responsibilities Lead and collaborate with the care team to manage and deliver patient care plans. Support patients in the Highland Park, Northbrook, and surrounding areas. Maintain accurate and timely charting to meet critical deadlines. Self‑manage time and schedule to provide effective patient care. Utilize tablets, mobile phones, and EMR software to document and coordinate care. Qualifications RN license in the state of employment. Graduate of an approved school of practical nursing. At least one year of RN experience and clinical competence to deliver quality patient care. Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations. Commitment to consistently meet critical deadlines for charting.
