Primary Care Provider: Nurse Practitioner or Physician Assistant

Responsibilities In a manner consistent and supportive of our values, the Primary Care Provider is responsible for delivering high quality health care within Fast Pace Health’s scope of services while achieving optimum patient satisfaction. The Primary Care Provider is responsible for preventative care for the patients as their first point of contact and takes continued responsibility for providing the patient’s ongoing comprehensive care, focusing on their long-term health and well-being. The Primary Care Provider must be able to work in a team-oriented environment, be flexible to adapt to new technologies and protocols in a quickly evolving practice setting. They must have the ability to respond quickly and accurately to changes in condition or response to treatment and is responsible for providing outstanding patient service within the clinic and through various virtual communication channels, while maintaining a compassionate and welcome atmosphere. Primary Care Essential: Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up. Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma. Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings. Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs. Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans. Prescribe medications, recommend treatments, and provide written home-going instructions. Refer patients to specialists or other healthcare resources as needed. Educate patients and their families on health maintenance and preventive care. Promote patient engagement, health literacy, and shared decision-making. Participate in quality improvement initiatives and value-based care programs. Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships. Essential Functions: Ability to provide quality care and meet patient volume goals, with an expectation of +18 daily on average. Preferred experience working as a Telehealth Clinician. Lead clinic staff in behaviors, actions and attitude (e.g., X-Ray Technologists, Nurses, Front Office Specialists) in delivering excellent patient care. Provide guidance as necessary to ensure quality professional services and patient satisfaction. Discuss and review patients’ medical history, symptoms, allergies, and current medications; ask situation-specific questions to formulate accurate diagnoses and provide guidance to ensure quality professional services and patient satisfaction. Engage with clinical leadership and escalate to management where appropriate to ensure strong patient care and alignment with company values. Ensure accurate completion of patient charts in a timely manner and forward charts as appropriate on a daily basis. Ability to work efficiently in a fast-paced autonomous environment. Dedication to evidence-based medicine and staying current with medical advancements. Implement clinical protocols as outlined by the Chief Medical Officer and Supervising Physicians. Ability to perform responsibilities within standard NP protocols. Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses. Deliver excellent patient care through in-person diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information. Ensure that the activities of the Primary Care Provider are conducted in a manner that is consistent with department expectations and compliant with Federal and State regulations and requirements, including HIPAA. Complete mandatory compliance training to meet and exceed ongoing quality of care requirements.
