Youth Community Support Specialist - Empower Recovery Skills

Betty Jean Kerr Peoples Health Centers currently has an opportunity for a Youth Community Support Specialist (CSS). This role focuses on activities and interactions to help individuals at clear risk of hospitalization or recently discharged to maintain their community living. The CSS performs assessments, contributes to person-centered plans, monitors progress, provides living and social skills training, and acts as a 24-hour resource when needed. #J-18808-Ljbffr Betty Jean Kerr People's Health Centers
