Collaborate with a dedicated team in a supportive environment.Job DescriptionSubacute Manager OverviewThe Subacute Manager oversees daily operations of a 25-bed subacute rehabilitation unit, ensuring compliance with federal, state, and local standards.Collaborates with the Director of Nursing to maintain high-quality care and 24-hour service coverage for residents.Plans, organizes, and evaluates nursing services to meet the needs of residents effectively.Ensures sufficient staffing levels, including RNs, LPNs, CNAs, and HHAs, for optimal care delivery.Monitors electronic charting systems for accuracy and compliance with facility policies.Participates in quality assurance programs to uphold the highest standards of care.Provides leadership and supervision to nursing staff, promoting professionalism and team cohesion.Adapts to changing circumstances and effectively manages crises and emergencies.Develops and implements nursing policies and procedures in alignment with regulatory requirements.Subacute Manager Key Responsibilities & DutiesAssist the Director of Nursing in managing nursing department functions and ensuring compliance with regulations.Schedule and supervise nursing staff, ensuring adequate coverage for resident care needs.Participate in care planning, including reviewing and updating resident care plans as needed.Investigate incidents and accidents thoroughly, ensuring comprehensive documentation and timely resolution.Monitor and ensure prompt responses to the Nurse Call Alert System by nursing staff.Conduct performance evaluations and administer disciplinary actions fairly and effectively.Participate in quality assurance and other programs to enhance care quality and compliance.Provide training and ensure staff proficiency in person-centered care practices for residents with dementia.Collaborate with other departments to maintain a safe and efficient facility environment.Subacute Manager Job RequirementsBachelor of Science in Nursing (BSN) and a valid New Jersey RN license.Minimum of 2 years of experience as a Nursing Supervisor in assisted living or long-term care facilities.Proficiency in electronic charting systems, including EMAR and ETAR, is advantageous.Strong leadership and team-building skills, with the ability to manage and supervise nursing staff effectively.Adaptability to changing circumstances and ability to handle crises and emergencies under pressure.Commitment to providing high-quality, person-centered care for aging residents.Physical ability to perform essential duties, including lifting and moving supplies and equipment.Willingness to work beyond normal hours, including weekends and holidays, as necessary.Excellent communication skills for interacting with residents, families, and healthcare professionals.
