Hamilton Park Center for Nursing and Rehabilitationis looking for a Medicaid Coordinatorto join the team at our Skilled Nursing Facility in Brooklyn, NY!

Medicaid Coordinator Responsibilities
• Screen, interview, andassess each resident to determine Medicaid benefit eligibility
• Receive and reviewadmission forms for residents referred for Medicaid assistance
• Collect all necessarydocuments to apply for Medicaid
• Upload documents to besent electronically to the Medicaid office
• Correspond with HRAcase workers to get the status of Medicaidapplications
• Follow up to ensurethat all applications are approved
• Obtain necessaryinformation to determine eligibility for financial aid
• Secure and completeappropriate forms in accordance with the establishedprocedure
• Perform otherjob-related duties as directed by Supervisor

Medicaid Coordinator Requirements
• High school diploma orits equivalent
• COVID-19 Vaccination

Preferred
• Bachelor's Degree inBusiness, Accounting/Finance, Marketing, Social Work, or a related field
• Previous experience ina long-term care facility or another healthcare-related facility
