Physical Therapist Assistant, Home Care, Bronx

VNS Health, a leading provider of home and community-based care in New York City, is seeking a dedicated Physical Therapist Assistant (PTA) to join our Home Care team serving patients in the Bronx. In this role, you will work under the supervision and direction of a licensed Physical Therapist to deliver high-quality, patient-centered rehabilitation services to individuals in their homes. You will implement established treatment plans, provide therapeutic exercises and functional mobility training, and assist patients with gait, balance, and strength activities to improve safety and independence in daily living.

As a PTA with VNS Health, you will visit patients across diverse Bronx neighborhoods, performing thorough observations of patient status, reporting changes in condition to the supervising Physical Therapist and interdisciplinary team, and documenting care in accordance with agency, state, and federal guidelines. You will educate patients and caregivers on home exercise programs, safe transfers, use of assistive devices, fall prevention, and energy conservation, tailoring instruction to each patient’s goals, cultural background, and home environment. You will collaborate closely with nurses, therapists, social workers, and physicians to support coordinated, holistic care plans. You will manage a daily home-visit schedule, prioritize patient needs, and maintain accurate, timely electronic documentation.
