Responsibilities
- Conduct comprehensive physical therapy evaluations for residents and develop individualized treatment plans to address mobility, strength, balance, and functional independence.
- Implement evidence-based PT interventions and modalities appropriate for geriatric populations in a SNF setting.
- Reassess patient progress regularly, document outcomes, and adjust goals and treatment plans accordingly.
- Maintain accurate, timely documentation of evaluations, progress notes, discharge plans, and other required records in the clinical information system.
- Collaborate with physicians, nursing staff, and the broader rehab team to coordinate care and optimize patient outcomes.
- Educate residents and families/caregivers on home exercise programs, safety, fall prevention, and progression of activities of daily living.

Requirements
- Active physical therapy license in New Jersey (or eligibility for licensure)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Experience in physical therapy, with preference for skilled nursing facility or geriatric rehabilitation settings
- Ability to work in a skilled nursing facility environment and to travel for the assignment
- Strong communication and documentation skills; comfortable using electronic medical records
