Job Description

Provide evidenced-based care to a panel of patients in collaboration with a dedicated clinical care team.
Conduct accurate assessment, diagnosis, treatment, and management of health problems.
Perform health counseling and disposition planning for patients.
Work at least 2-3 days per week, from 8:30 AM to 7:30 PM.
Participate in holiday rotations as needed.

Critical Information

Onsite as needed; this is an in-person, patient-facing role.
Training is 4 ten-hour shifts for four weeks, with flexibility.

Education/Licenses Needed

Active and in good standing Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of application.
Current Basic Life Support (BLS) certification from American Red Cross (ARC) or American Heart Association (AHA).
High School Diploma or GED required.
