Physical Therapist Assistant (PTA) - Home Health

Help Patients Regain Strength and Independence

We are looking for a dedicated and compassionate PTA to join our growing home health team. Working under the supervision of a PT, you will provide individualized therapy services that helps patients improve mobility, reduce pain, and safely regain independence in the comfort of their own homes. Responsibilities

• Provides physical therapy treatments according to the established plan of care under the direction of a licensed PT
• Assists patients with therapeutic exercises, gait training, balance activities, transfer training and mobility techniques
• Monitors patient progress and communicate changes in condition to the supervising PT and care team
• Educates patients and caregivers on home exercise programs, fall preventions, and safe mobility techniques.
• Completes accurate and timely clinical documentation
• Collaborates with physicians, nurses, occupational therapists, speech-language therapists and other healthcare professionals to ensure coordinated patient care
• Maintains compliance with agency policies and state and federal home health regulations

Qualifications

• Current Ohio Physical Therapist Assistant license
• Current CPR and/or BLS certification
• Valid driver's license, reliable transportation and proof of auto insurance
• Strong communication, organizational and interpersonal skills
• Home health experience is preferred, but not required
