RN Ambulatory Nurse Mason Square Community Health Family Medicine

Baystate Health western Massachusettss premier healthcare provider and home to Baystate Medical Practices is looking for an experienced Registered Nurse to join our Family Medicine Community Health Centers !

• Functions independently to assess develop and implement plan of care for complex patients including medical and/or psychosocial disorders.
• Provides independent quality triage and patient care utilizing strong assessment skills point of care testing and quality assurance checks.
• Establishes plan of care sets priorities and implements the plan of care with patient/family and team/provider input.
• Builds on knowledge and experience to include more diagnoses and complicated medical patients.
• Triages and coordinates patient care ensuring access to care and patient success.
• Promotes health and wellness for individuals and groups of patients.
• Demonstrates a proactive approach to acute and chronic disease management.
• Demonstrates respect and advocates for the patients rights values and belief systems.
• Develops patient trust and engages the patient/ family in care and social and cultural differences that might impede patient care

Requirements

• Massachusetts license as an RN and BLS CPR certified.
• Minimum of 2years of nursing experience
• Experience in an outpatient primary care or health center setting preferred
• Ability to work in a fast-paced environment & handle multiple tasks/priorities
• Excellent verbal and written communication and interpersonal skills.
• Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians and staff

• Full Time 40 hours hours Monday - Friday 9-5:30PM
• 11 Wilbraham Road Springfield MA
• Sign On Bonus eligible for external with 1 year of experience

Sign on Bonuses are paid to external hires only; not applicable for internal or company transfers unless otherwise specified. Former employees are eligible after 1 year post separation from Baystate Health .

Compensation

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidates relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.
