Job Description - NURSE HOME VISITOR WIREGRASS Region (Covington, Coffee, Geneva, Dale, Henry, Houston counties) (T232012) NURSE HOME VISITOR WIREGRASS Region (Covington, Coffee, Geneva, Dale, Henry, Houston counties) - ( T232012 ) Description The University of Alabama at Birmingham (UAB) is seeking a NURSE HOME VISITOR WIREGRASS Region (Covington, Coffee, Geneva, Dale, Henry, Houston counties) to provide comprehensive nursing services to women and their families eligible for the Nurse-Family Partnership (NFP) Program. To maintain the highest standards of clinical nursing practice and adherence to NFP model and to policies, procedures, guidelines and standards of the NFP and of the lead agency. RESPONSIBILITIES Adhere to nursing process and the NFP model of home visitation. Provide home visits to women and their families eligible for the NFP Program. Carries a caseload of up to 30 clients. Support policies, procedures, guidelines, and standards of NFP and the lead agency. As requested, /required, participates in community coalition in support of NFP, promotes public awareness and develops/maintains community relationships to support client referrals. Complete all required NFP Education Develop therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment. Perform home visiting in accordance with the NFP Model Elements and NFP Visit-to-Visit guidelines. Follow nursing process in implementing NFP into nursing practice. Accesses physical, emotional, social, and environmental needs of women and their families as they relate to the NFP domains. Provides education, support and referral resources in assisting women and their families in attaining their targeted goals. Perform other duties as assigned. Qualifications Graduate of an accredited Nursing program required. Bachelor's degree in Nursing or a related field required, or a combination of education and experience. Currently licensed or licensed by endorsement as a Registered Nurse (RN) by the Alabama Board of Nursing required.
