Rural Health Transformation Program Evaluation

The State of New Hampshire needs an external evaluation partner to oversee their Rural Health Transformation Program (RHTP) — a CMS-funded statewide initiative. The vendor will evaluate five health programs running across New Hampshire, monitor performance data monthly, report findings to state leadership and CMS, and help course-correct when programs are not on track. This is an evaluation and advisory services contract — NOT a staffing placement contract. Contract value is up to $25 million through 2031.

Rural Health Subject Matter Expert

What they will do: Provide rural health context across all evaluation activities; advise on care coordination, telehealth, mobile health, pharmacy access, and NEMT Minimum experience needed: - 6+ years direct rural health program, policy, or services experience - Knowledge of Critical Access Hospitals, FQHCs, or Rural Health Clinics - Familiarity with CMS rural health programs NH or New England experience is HIGHEST priority for this role
