Medical Assessment Coordinator

Are you a compassionate and dedicated to helping others? Join our team and play a vital role in supporting the emotional and cognitive health of residents in long-term care settings. We are seeking a Clinical Support Specialist to conduct BIMS (Brief Interview for Mental Status) and PHQ-9 (Patient Health Questionnaire) screenings, ensuring residents receive the comprehensive care they deserve.

What You'll Do:

Conduct BIMS and PHQ-9 screenings with residents in compliance with federal and state guidelines.
Collaborate with interdisciplinary teams to evaluate and support residents' cognitive and emotional well-being.
Provide insight and documentation that informs care planning and improves residents' quality of life.
Maintain accurate records and adhere to confidentiality standards.
Build trusting relationships with residents and facilities, helping to foster a supportive community.

What We're Looking For:

Previous experience in long-term care, skilled nursing, or geriatric setting.
Familiarity with BIMS, PHQ-9, and resident assessment protocols (MDS).
Excellent interpersonal, communication, and documentation skills.
Empathy, patience, and professionalism in every interaction.
