Job Description

The On-Call Hospice LVN provides after-hours nursing support and patient care for hospice patients in home and facility settings under the direction of the Registered Nurse and interdisciplinary team. This role responds to urgent patient and family needs, provides symptom management support, reinforces education, and helps ensure comfort and continuity of care during evenings, nights, weekends, and holidays as assigned.

This position requires strong communication skills, prompt response times, and the ability to work independently while delivering compassionate end-of-life care.

Responsibilities

• Answer and manage after-hours hospice phone calls while on-call
• Respond to patient, family, and facility concerns in a timely and professional manner
• Perform on-call and PRN nursing visits as needed
• Assess patient condition and report significant changes to supervising RN and provider
• Assist with symptom management and comfort-focused interventions
• Reinforce education regarding medications, disease progression, safety, and end-of-life care
• Provide emotional support and therapeutic communication to patients and families
• Accurately document all patient interactions and visits in the EMR
• Collaborate with the hospice interdisciplinary team to ensure continuity of care
• Maintain compliance with hospice regulations, policies, and standards of care

Qualifications

• Current Texas LVN license in good standing
• Minimum 1 year nursing experience required
• Hospice, home health, long-term care, or palliative care experience preferred
• Strong assessment, communication, and critical thinking skills
• Ability to work independently in the field
• Reliable transportation and valid Texas driver’s license
• Current CPR certification required

Compensation

• $150 weekend on-call coverage stipend
• $50 per 12-hour on-call shift
• $35 per patient visit completed if called out during on-call coverage

Schedule

• PRN / As Needed
• Primarily evenings, nights, weekends, and holiday on-call coverage
