Case Manager

Job Summary

The Tribal Case Manager provides direct support to Tribal Social Services through case management, advocacy, transportation assistance, referrals, community outreach, and coordination of services for tribal members and families. This position reports to Tribal Social Worker/ Director of Social Services to improve access to resources, strengthen service delivery, and support the health and well-being of the community. The Tribal Case Manager plays an important role in helping community members navigate social service systems while assisting with daily departmental operations and community support needs.

Essential Duties and Responsibilities
  • Assist tribal members and families with accessing social services, community resources, and support programs.
  • Provide case management services, follow-up support, and advocacy for clients.
  • Assist with referrals and coordination with outside agencies and service providers.
  • Support community members with applications for assistance programs and benefits.
  • Provide transportation assistance for appointments, services, and approved community needs when necessary.
  • Conduct outreach and maintain regular communication with clients and families.
  • Assist with crisis response and stabilization efforts as directed.
  • Maintain accurate case notes, records, and documentation.
  • Protect confidential and sensitive information in accordance with tribal, state, and federal regulations.
  • Assist with departmental projects, reporting requirements, and community initiatives.
  • Participate in trainings, meetings, and professional development opportunities.
  • Perform other duties as assigned.
Minimum Qualifications
  • Experience working in social services, behavioral health, case management, or community programs.
  • Ability to work effectively with individuals and families experiencing crisis or other social service challenges required.
  • Valid driver's license and insurable driving record required.
  • Ability to maintain confidentiality and professional boundaries.
  • Strong communication, organization, and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to establish and maintain effective working relationships with tribal members, staff, partner agencies, and the public.
  • Must pass a drug screen and background check.
Preferred Qualifications
  • Bachelor's degree in social sciences or closely related field
  • Experience working with Native American communities and cultures.
  • Knowledge of community resources and service systems.
  • Experience providing transportation, outreach, or community-based services.
Physical Requirements
  • Ability to sit, stand, walk, bend, and lift up to 25 pounds.
  • Ability to travel locally and transport clients when necessary.
  • Ability to work occasional evenings or weekends for community events or emergencies.
