Who You Are The RN Case Manager cares for patients with terminal illnesses or other diagnoses, indicating they have fewer than six months to live. The RN Case Manager will provide end-of-life care to ensure patients are as comfortable as possible and have the highest possible quality of life. The RN Case Manager is typically part of a care team. This team collaborates to ensure individuals, and their families have the physical, emotional, and spiritual support they require throughout the care process.
