Job Title: RN Case Manager Profession: RN Specialty: Home Health RN Duration: 13 weeks Shift: Days Hours per Shift: 36 Experience: 2 years License: Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license. Certifications: Graduate of an accredited school of nursing. Must-Have: 2 years of experience in Home Health Description: This position offers the opportunity to work as an RN Case Manager in a home health setting. You will have the chance to get to know your patients and become an integral part of their care team. The role provides broad independence while being part of a cohesive patient care team. You will enjoy a work/life balance with the flexibility you need. As an RN Case Manager, you will screen, evaluate, re-evaluate, and treat patients. You will coordinate and communicate treatment activities with interdisciplinary team members and patients. The RN Case Manager develops a care plan in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission. You will periodically review and recertify the patient care plan as appropriate. Coordinating the patient's care plan through interdisciplinary communication and case conferences is a key responsibility. You will advise other disciplines of discharges in a timely manner. Ongoing assessment of the patient's clinical, functional, and emotional status will be required to identify problems and/or progress toward desired goals. Significant changes will need to be communicated to the patient's physician and colleagues. Documenting evaluations, treatment goals, and objectives is essential. Teaching self-care techniques to the patient and caregiver as appropriate will be part of your role. You will provide medication, diet, and other instructions as ordered by the physician and utilize opportunities for health counseling with patients and caregivers.
