Short Description

The case manager provides crisis intervention, consultation, referrals, and brief interventions to patients. The role also completes follow-up with patients to assist in treatment linkages and navigating complex social service applications such as housing applications, disability enrollments, etc.
Experience Required

Previous acute care experience preferred.

Education Requirements

Bachelor Degree -Social Work/Registered Nurse required.

License/Certification Requirements

• RN licensure in the State of New Jersey.

Hourly Rate Min $34

Hourly Rate Max $56
