Highmark Inc. is seeking an Outreach Representative to serve as a liaison between members, the Health Plan, and regulators. The role includes telephonic support, scheduling, educating members on rights and responsibilities, and assisting with grievance and appeals processes. You will initiate outreach for preventive health, recruit and enroll members in Disease Management Programs, and coordinate care management referrals while tracking contacts in information systems. #J-18808-Ljbffr Highmark Health
