The Families FIRST program at Terros Health assists families involved with the Department of Child Safety. Working as part of a team, the Care Coordinator will impact the lives of families trying to overcome the barrier of substance abuse to reach family reunification and self‑sufficiency. This individual will work closely with DCS, the clinical team and other providers to coordinate and help families be successful in their DCS case plans. Part of their responsibility is to provide monthly meetings to assure that goals are being set and achieved. They will also empower families by providing resources and skills to show stability in their homes. Terros Health is hiring a Families FIRST - Care Coordinator at our Olive Ave Health Center in Glendale, AZ. *Requires Travel Outside of Maricopa County* Full-Time, Employed Mon - Fri, 9:00am - 5:00pm Flexible Scheduling Full Benefits, Including 401K, and Generous PTO/PST. Under Terros Health’s policies and professional requirements, with oversight from clinical staff, the Families FIRST Care Coordinator works closely with DCS, collaborative partners and other community resources to engage patients and streamline services to remove barriers, alleviate substance use and improve the prospect of family reunification. This position reports to the Manager of Care Coordination Essential Duties Accepts and manages Arizona Families FIRST (AFF) referrals assigned to Terros Health in service areas outside of Maricopa County. Completes initial outreach to referred families, coordinate with DCS Specialists and other involved stakeholders, and facilitates engagement in AFF services. Travels throughout assigned geographic regions to conduct in-home assessments, completes intake and assessment documentation, gathers required information, and determines service needs. Develops short-term service recommendations and coordinates timely linkage to behavioral health, substance use treatment, recovery support, and community-based services. Coordinates ongoing AFF services through approved subcontracted providers, community partners, telehealth resources, and/or Terros Health programs as appropriate. Maintains responsibility for ongoing care coordination throughout the episode of care, ensuring services align with identified needs and DCS requirements. Develops, implements, and monitors individualized Service Plans within required AFF and DCS timelines. Facilitates multidisciplinary meetings and collaborates with patients, family members, DCS Specialists, treatment providers, and community partners to review progress, identify barriers, and support service plan goals. Participates in Child and Family Team (CFT), Adult Recovery Team (ART), Team Decision Making (TDM), SENSE, DCS staffing meetings, and other collaborative meetings as required. Knowledge of child welfare, child safety, behavioral health, substance use disorder treatment, and community-based service systems. Experience coordinating services among multiple stakeholders, including families, community providers, and government agencies. Strong assessment, case management, care coordination, and problem‑solving skills. Effective verbal and written communication skills with the ability to engage diverse populations and collaborate with multidisciplinary teams. Demonstrated ability to manage competing priorities, maintain organization, and meet established deadlines and productivity expectations. Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and Teams. Experience utilizing Electronic Health Records (EHRs); NextGen experience preferred. Ability to accurately complete documentation and maintain records in accordance with agency, contractual, and regulatory requirements. Ability to work independently while traveling throughout assigned service areas and maintaining communication with supervisors and team members. Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
