Social Services Director / PRSD

As a Social Services Director / PRSD at Oak Park Oasis, you will play a pivotal role in delivering exceptional care to our residents, ensuring seamless transitions, and promoting independence. Located in Oak Park, Illinois, we are a Joint Commission Approved facility dedicated to providing high-quality services that exceed patient expectations.
About the Role
As a Social Services Director / PRSD, you will be responsible for:
Developing and implementing individualized care plans that address the social, emotional, and psychological needs of our residents
Conducting comprehensive assessments to identify and address any barriers to discharge and develop strategies to overcome them
Collaborating with our interdisciplinary team to ensure continuity of care and effective communication with residents, families, and healthcare providers
Coordinating outside services and resources to support the transition of residents back to their homes
Monitoring and evaluating the effectiveness of care plans and making adjustments as needed
Master's Degree required
Responsibilities
Develop and implement policies and procedures for social services, ensuring compliance with regulations and accreditation standards
Supervise and mentor social work staff, providing guidance and support to ensure high-quality services
Develop and maintain relationships with community resources, healthcare providers, and other stakeholders to ensure effective referral and discharge planning
Participate in quality improvement initiatives to reduce re-hospitalization rates and improve patient outcomes
Collaborate with our medical director and other healthcare professionals to ensure comprehensive care
