Setting: Home Health & Hospice Care
Schedule: Monday-Friday | 8:00 AM - 5:00 PM
Position Summary:
We are seeking a compassionate Nurse Practitioner to serve as the Hospice Clinical Director while providing high-quality, patient-centered care in home health and hospice settings. This role involves clinical leadership, patient assessments, treatment management, and collaboration with interdisciplinary teams to ensure optimal patient outcomes.
Responsibilities:
• Conduct patient assessments and evaluations
• Prescribe medications, manage treatment plans and develop individualized care plans
• Monitor patient progress and adjust care as needed
• Educate patients and families regarding care and treatment options
• Ensure compliance with hospice and home health regulations
• Advocate for quality patient care and positive outcomes
Qualifications:
• Current and unrestricted Idaho Nurse Practitioner license
• Experience in hospice, home health, or rural healthcare settings preferred
• APRN Certification and BLS Certification
• Ability to prescribe medications and manage treatment plans
• Knowledge of home health and hospice care practices
