Position Summary:
Advanced nursing practice is defined as the performance of advanced level nursing practice activities that, by virtue of post basic specialized education and experience, are appropriate to and may be performed by an APRN. (Conn. Gen. Stat. 20-87a (b)). For the first 3 years after having been issued a license and no less than 2,000 hours of practice, NPs must perform acts of diagnosis and treatment of alterations in health status in collaboration with a physician. The collaboration must address a reasonable and appropriate level of consultation and referral, coverage for the patient in the absence of the APRN, a method to review patient outcomes and a method of disclosure of the relationship to the patient. The collaborative agreement must be in writing and include the level of schedule II and III drugs that the NP may prescribe and a method to review patient outcomes. (Conn. Gen. Stat. § 20-87a
Essential Job Functions:
Provides clinical care within scope of Nurse Practitioner license.
1.
Assesses patient illness by obtaining complete medical history ensuring that findings are properly recorded in the patient's medical record. Performs physical examination, orders laboratory tests, and analyzes results.

Diagnoses illness or health needs based on analysis and evaluation of clinical data and physical assessments.
3.
Provide direct patient care as a member of the clinical team working with
Nurses, MD's, Medical Assistants, and Case Managers and Patient Access Representatives toward achieving the goals for the patient's plan of care.
4.
Collaborates with team members to assure quality patient care, appropriate distribution of work, and efficient patient care.
5.
May concentrate in such medical specialties as appropriate to skills and Board certifications.

6.
Participates in rotation to provide services to the community and participate in health fairs as needed.
7.
Provide patient care and treatment in collaboration with a physician in accordance with established state laws and organizational job description.
8.
Refers patients for diagnostic tests and specialist services as appropriate.
9.
Works with other staff to plan, develop and evaluate patient care plan and advocates for and supports community health programs and policies.
10.
Participates in Professional Practice, Quality Improvement, Staff, and other meetings as assigned.
11.
Maintains and enhances professional development and knowledge of theory and practice by attending conferences, educational programs and seminars.
12.
Other duties as requested.
Performance Goals:

Ability to effectively track client data, specialty appointments and diagnostic tests.

Aware of resources, referral options and services outside of the CHWC
Additional General Requirements: Professional positive attitude, understanding of customer service principles, trustworthiness, and excellent interpersonal skills.
Job Qualifications/Requirements: Current APRN license. CPR certification. Mandatory Education, Case Review, and Departmental Meetings as outlined by the Medical Director.
Education:
Position requires a master's degree from an accredited program in nursing and successful completion of an accredited certified nurse practitioner program.
Experience: This position requires experience as a nurse practitioner in a Community Health Center/Primary Care setting, or an Emergency Department/Urgent Care. Family APRN training preferred with Pediatric experience, however, will consider other subspecialty training and experience.
