Location: Woodbury, NJ Contract Term: March 1, 2026 February 28, 2027 (1 Year) Submission Deadline: December 9, 2025 2:00 PM EST Estimated Hours: ~40 hours / month Work Arrangement: Onsite, weekly coordination meetings required To appoint a licensed, experienced Dietitian to provide nutritional consulting services for Shady Lane Nursing Home residents and staff, ensuring compliance with all federal, state, and facility regulations governing long-term care nutrition management.

The Dietitian Will:

• Review residents' medical and nutritional information to assess dietary needs.
• Develop, initiate, and update nutritional care plans for new admissions, re-admissions, significant changes, quarterly, and annual reviews.
• Approve and modify monthly menus in collaboration with the Food Service Director.
• Provide nutrition education to residents and quarterly staff in-services.
• Collaborate weekly with Food Service Director and Nurse Manager to address dietary concerns and compliance issues.
• Maintain accurate documentation in the Sigma Care System and ensure survey readiness.

Key Responsibilities

1. Resident Care & Nutrition Planning
   • Review medical records to determine individualized nutritional needs.
   • Implement and update resident care plans for all new/re-admissions, significant changes, quarterly and annual assessments.
   • Document recommendations and therapeutic diet orders in the Sigma Care System.
2. Menu Management & Food Service Oversight
   • Review and approve menus monthly; make modifications based on residents' needs.
   • Meet weekly with the Food Service Director to discuss menu adequacy, compliance, and resident satisfaction.
   • Recommend changes ensuring nutritionally balanced and medically appropriate meals.
3. Education & Training
   • Provide quarterly in-service education to staff on nutrition, hydration, and intake management.
   • Educate residents about therapeutic diets and nutrition practices that enhance health and quality of life.
   • Submit an annual in-service calendar to the Director of Nursing (DON) at the beginning of each contract year.
4. Reporting & Compliance
   • Submit written dietitian reports to: Food Service Director, Director of Nursing, Nurse Manager, MDS Coordinator.
   • Be on-call during survey periods and available for face-to-face meetings with survey teams.
   • Ensure compliance with state and federal long-term care nutrition standards.

Education, Experience & Qualifications

• Licensed Dietitian (New Jersey) Required
• Bachelor's or Master's degree in Dietetics, Nutrition, or related field Required
• Experience in long-term care or rehabilitation settings Required
• Knowledge of Medicare/Medicaid nutrition documentation and compliance Preferred
• Familiarity with Sigma Care System or equivalent EHR platforms Preferred
• Strong interpersonal, communication, and documentation skills
