Integrated Acute Care Coordinator RN

The purpose of the Acute Care Service Coordinator role is to partner with the IAC Medical Director in building one world-class integrated acute care team across the care continuum which creates safe & controlled care transitions and implements high functioning and cost-positive protocols & quality measures. The role is composed of three fundamental capabilities: practice management, opportunity identification, and model implementation / change acceleration.

Essential job functions include supporting the Medical Director in deploying a well-structured site operating rhythm for the IAC team, working with site administration and clinical team to identify, plan, and drive improvements in a project management environment, and acting as an agent of change and becoming the hospitals point of contact for difficult-to-manage situations.

Knowledge, skills, and abilities required include execution, clear and concise communication, business impact/challenge and out-of-the-box thinking, and leadership.

Education and experience required include a BSN, five or more years of relevant work experience with emphasis on performance improvement, data and project plan management, and clinical experience in emergency/outpatient, inpatient, and post-acute settings.
