Clinical Nurse Navigator I

The Clinical Nurse Navigator I is a registered nurse responsible for the seamless coordination of care across the continuum for patients with complex, integration-sensitive diagnoses. The Navigator acts as a patient advocate coordinating care in conjunction with physicians from the time of diagnosis or admission through discharge or end of life. The role includes educating patients and families/significant others on the healthcare experience. This position implements, coordinates, monitors, and evaluates care options, services, and resources. The Navigator links families/patients to appropriate resources as needed. The role is proactive in finding ways to enhance the patient experience both clinically and in service. The Navigator provides an exceptional patient experience. Additionally, this position assists in quality improvement initiatives by gathering, tracking, and researching quality metrics. It monitors outcomes such as patient satisfaction, compliance with treatment regimen, and loyalty. The Navigator interacts with department nurses and may orient and train incoming clinical nurses and patient care assistants. Other duties may be assigned as needed.

Experience: Minimum of five (5) years in nursing and three (3) years in the area of specialty License: Registered Nurse license in the State of California - Current and in good standing Certifications: Not Specified Must-Have: - Five (5) years in nursing - Three (3) years in area of specialty - Client: Fulfills mandatory stroke education requirements per certification agency - Oncology: One (1) year of Oncology nursing experience
