Our 110-bed facility houses a resident demographic from varying acuity, giving you the opportunity to assist in the betterment of all walks of life amongst those we steward.Work schedule: Full-time and PRNPay rate: $37.00 - $42.00 / hour – depending on experienceJob summary:This position ensures that quality care is delivered through monitoring of staff performance, physician contact, care planning, and daily resident observation. This position also provides direct care to residents. All this is done according to nursing standards of practice, standard procedures, and established state and federal regulations for skilled nursing facilities.Job duties:Assist routine daily direction and monitor the work of the nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks.Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.Communicate on a regular basis with the Nurse Manager or Director of Nursing regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.Participate in report at the beginning and end of shifts to assure proper information about residents is communicated.Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.Assist in the admission, orientation, and observation of new residents, assure that physician orders for diets, therapy, medication, etc. are transcribed and communicated according to procedure.Qualifications / education / experience:Currently licensed as an LPN in ColoradoPrior experience in a nursing communityThe ability to read, write, speak, and understand the English languageThe desire to care for residents and act with integrityThe ability to work flexible hoursPossess a valid BLS CertificationAffirmative action statement: Highland Park Rehabilitation Skilled Nursing Facility is an equal opportunity employer committed to fostering a diverse and inclusive workplace.
