Lead Integrated Emotional Wellness Clinician

Washington, DC | On-Site | $80K - $90K | Washington Post Top Workplace (8x Winner) Community of Hope is seeking a Lead Integrated Emotional Wellness Clinician to join our cohesive medical team to provide brief assessments, screenings, crisis intervention and short‑term counseling services which will reduce barriers to mental health care and educate patients and staff about mental health and treatment options. What You’ll Do Leads the initiatives to increase capacity and expertise surrounding perinatal mental health needs and services, including program design and implementation, and data evaluation. Supports and closely coordinates mental health care with the patient’s primary care team and when appropriate, coordinates with other treating mental health providers. Screens and assesses patients for common mental health and substance use disorders. Consults with supervisor on difficult to engage clients. Educates patients about common mental health and substance use issues and available treatment options. Helps identify and address related problems affecting health of client, such as housing, vocational rehabilitation, and social integration, and coordinates service delivery with other health center staff and community providers. Provides crisis intervention per the clinician’s licensing and COH guidelines, to include addressing suicidal/homicidal ideations. Provides short‑term, problem‑focused therapy for no more than 5 patients, unless approved by the supervisor and for up to 8 sessions per patient as deemed appropriate. Short‑term therapy will focus on goal setting, change, and utilizing brief interventions. Maintains a daily schedule of at least 6 patients seen per day. Completes a treatment plan for all short‑term therapy clients by the 2nd scheduled session. Monitors client progress through the course of treatment, reevaluating and adapting the treatment plan at least monthly from initial treatment plan date. Collaborates with PCPs and psychiatric nurse practitioners to support patients who are not improving as expected. This collaboration may include treatment plan changes including new medications or psychosocial treatments, or appropriate referrals for additional services. Facilitates and appropriately documents referrals for clinically indicated services outside the primary care clinic (e.g., domestic violence safe shelter, mental health specialty care, core service agency, and substance use treatment). Provides leadership of emotional wellness integration with the midwifery and maternal and child health teams, ensuring that activities align with COH policies, procedures, and strategic goals. Meets with supervisees regularly and provides real‑time feedback, training, support, and coaching as needed. Creates meeting agendas, sends meeting agendas in advance when possible, and documents concerns promptly as needed. Performs other tasks as requested by supervisor. Requirements Must-Haves LICSW, LPC, or equivalent licensure required. (Any providers who are licensed in Maryland who provide a copy of their license and MD CDS to Talent Management are also eligible for MD licensure reimbursement.) Minimum of two years of work experience in a clinical environment. At least two years of supervisory experience. Strong interpersonal and organizational skills. Demonstrated ability to collaborate effectively in a team setting. Experience in screening for common mental health symptoms. Experience with assessment and treatment planning for common mental health and/or substance use disorders required. Familiarity with brief, structured intervention techniques required. Demonstrated ability to multi‑task and problem‑solve. Ability to work with technology and electronic health records. Ability to travel between Community of Hope sites. Ability to work flexible hours, including evenings and weekends. Proof of required vaccinations is required. This includes, but may not be limited to, Flu and Covid.
