Title Nurse Practitioner - Advanced Practice Provider Location 1625 Rio Bravo Blvd SW Suite 36, Albuquerque, NM 87105 Role Summary Provide effective, equitable value‑based primary care to adults on Medicare, ensuring patients remain healthy and out of the hospital. Work independently and collaboratively within a multidisciplinary outpatient team, delivering longitudinal care, care coordination, and population health leadership.
