Key Responsibilities\n

• Conduct thorough psychosocial assessments utilizing interviews, observational techniques, and a comprehensive review of medical records to identify emotional, social, and environmental factors affecting patients' recovery.

• Assist patients in navigating community resources, which may include connecting them to support groups, vocational rehabilitation, or housing assistance to enhance their recovery journey.

• Educate patients and their families about available community resources and the array of post-rehabilitation services that can support their ongoing needs.

• Play an active role in discharge planning, facilitating smooth transitions to home or other care settings with appropriate follow-up support.

Compensation and Benefits\n

The salary for this full-time position ranges from USD 75,000 to 100,000 annually, with opportunities for bonuses.
