Social Worker-Abdominal Transplant Clinic

Job Overview Saint Luke's Abdominal Transplant team seeks a full‑time Licensed Social Worker to support both outpatient and inpatient Liver and Kidney Transplant departments. The role focuses on patient and family social work services, including counseling, crisis intervention, and discharge planning. Responsibilities Assess the psychological, social, emotional, cultural, and financial needs of patients and families during illness. Provide supportive counseling and clinical services for crisis intervention and post‑discharge planning. Collaborate with the health‑care team to develop transition plans and address patients’ needs promptly. Participate in program planning and development to meet the needs of patients within the health‑care system and community. Lead initial assessments of patients’ psychological needs and develop appropriate treatment plans. Work in a team environment, mentoring, precepting, and onboarding new staff. Qualifications Licensed Master Social Worker (LMSW) with a Master’s degree in Social Work. Less than one year of applicable experience in social work (outpatient experience preferred). Strong communication and collaborative skills. Ability to work Monday through Friday, 8am to 5pm, with occasional flexible scheduling. Why SaintLuke’s?
