Job Location and Scope This role focuses on Home Health Care in the Dearborn Heights and Allen Park area, with a steady referral source from multiple skilled nursing facilities. The role includes PTA supervision and offers the opportunity to pick up additional time at a nearby SNF. What is Take Your Therapist Home? Take Your Therapist Home (TYTH) is a unique program where clinicians work directly with patients in SNF and home health, providing continuity of care for greater impact on patient outcomes. Required Qualifications Successful completion of an accredited Physical Therapist program Licensure as required by the State of Michigan Skills and Abilities Professional judgment and initiative in analyzing problems and recommending solutions Initiative and flexibility in the work environmentAbility to respond effectively to patients and families Physical ability to bend, position, transfer, and lift patients weighing from 80 to over 300 pounds, using clinically recommended lifting devices Why Rivetus?
