Overview
Licensed Practical Nurse (LPN) needed for a travel assignment in a long-term care setting in Concord, New Hampshire. Start date: 08/10/2026. Duration: 15 weeks. Weekly pay: $1,250–$1,352. Guaranteed hours: 36 per week. Shift: 3x12-hour nights (7:00 PM–7:00 AM). This temporary assignment focuses on direct resident care, routine assessments, medication administration, and collaboration with the care team in a long-term care environment.

Responsibilities
- Provide direct nursing care to residents in a long-term care setting per individualized care plans
- Monitor vital signs, perform routine assessments, and report changes to the supervising nurse
- Administer medications and document administration accurately according to policy
- Assist residents with activities of daily living and rehabilitative activities as needed
- Maintain accurate and timely documentation in the chart or electronic medical record
- Collaborate with RNs, physicians, and the broader care team to meet resident needs and safety standards

Requirements
- Current Licensed Practical Nurse license with eligibility to practice in New Hampshire
- Availability to work night shifts (7:00 PM–7:00 AM) on a 3x12 schedule
- Experience in a long-term care or skilled nursing setting preferred
- Strong communication, observation, and documentation skills
- Ability to function effectively within a multidisciplinary team and maintain resident safety
